Please take all original and 1 set of photocopies
- Online registration certificate
- Passport for both partners
- Visa for both partners or PIO / OIC cards, if applicable
- SM-IP agreement. If your surrogate mother is different from the
- Present and permanent residential address documents
- Marriage Certificate. If not in English, please translate and get it notarized
- Letter from Embassy stating surrogacy is legal in their country (Sweden citizens needs letter from Consulate in Mumbai)
- Letter from Hospital, where your cycle is planned
- Letter from SurrogacyIndia about your cycle
- Confirmed tickets for parents
- Treating Doctor's Degree Certificate copy
